Griffith University is collecting the information on this form (which may include medical information) for the following purposes:

1. To process your child's registration to the State Honours Ensemble Program (the Program)
2. To administer the State Honours Program, and
3. To provide appropriate care for your child during the Program and in case of a medical emergency.

If you choose not to provide the requested personal information, we may not be able to process your application.
The personal information collected is confidential and will not be disclosed to third parties without your consent, unless we are authorised or required by law to disclose the information.

Medical Information

Whilst we encourage participants and staff not to bring allergen products (nuts etc.) into the venue or facilities we use, we are not in position to guarantee that the facilities will be allergen free. As many of the spaces are accessed by the public, or hired by other users, you will need to consider this if you choose to accept a position in the program. If you do chose to accept, it will be at your own risk.

I understand that Open Conservatorium staff will endeavour to contact me in an emergency and I have provided all relevant details of my child's medical or physical needs. If I am unable to be contacted I authorise the Open Conservatorium staff members to obtain medical assistance which they deem necessary should an accident or illness occur. I agree to pay all medical and ambulance expenses incurred on behalf of my child. (required)

Photograph and audio recording

The Griffith University State Honours Ensemble Program is seeking your consent to use and publish your child's image captured in photograph, audio-visual and voice recordings including for distribution to participating families and our promotional purposes.
Your child's image or recording may contain details of their face, physicality and appearance, gender, ethnicity and other characteristics. In some instances, our capture of images and recordings may be unable to exclude or filter-out your child (e.g., performances in a gathered group formation). However, if you do not consent, we will not use or publish images or recordings of your child - unless, as with group settings, the image or recording is obscured, redacted or otherwise does not identify your child.
The consent you provide below will be valid for the duration of your child's participation in the State Honours Ensemble Program.
You are free to revoke your consent anytime. Upon revocation, we will stop using and not publish your child's photos and recordings. We cannot, however, unpublish photos or recordings that have already been published to external forums that the University does not retain control over.
Our copyright of photographs and audio recordings - Copyright and privacy are not the same thing. Privacy is about how we collect and handle personal information (e.g., your child's images and audio recordings) in accordance with privacy principles. Copyright is about our ability to use, reproduce and share the images we capture as part of our cache of original works once you have consented below.

Terms and Conditions

I do hereby accept my offer for the State Honours Ensemble Program Queensland and agree to all conditions as stated in the offer email.
Participants choosing to leave for any period of time during SHEP, for any reason other than medical grounds, choose to withdraw themselves from the remainder of the program. I understand that refunds are generally not given for withdrawal from the program.
